Instructions

1. Two (2) most recent and original pay stubs or Leave and Earnings Statement or Offer letter on company letterhead.

2. A copy of applicant's driver's license, age of majority card or military I.D.

3. Proof of valid Social Security number or all applicants in the United States on a work VISA must provide a copy of the VISA.

4. Written Employment Verification Form signed by applicants and Employer.
